Care Coordinator
Salary: £21,640.00 – £24,595.00

On behalf of Care Enterprises Primary Care Networks, LiveWire are seeking to appoint a Care Coordinator.
The Care Coordinator role is to be funded for a period of 12 months with the possibility of a long term extension.
We are looking for an energetic employee who will have the ability to engage with a wide range of individuals.
The successful candidate will work in partnership with GP practices, the Community Matron, patients, care homes and other partners across Liverpool.
You will be required to have knowledge of 24 hour care setting within the local area and demographical insight, a level of understanding of the patients who reside in them.
Identifying any gaps in delivery, you will be expected to support the Community Matron.
You will receive all the resources and training you need and will be fully supported to be excellent in this role.
