National Diabetes Prevention Programme
The NDPP is a national scheme set up by NHS England. It is a nine month behaviour change programme aimed at supporting people who have been identified as at risk of developing type 2 diabetes ( also known as pre-diabetic). It is a tailored healthy nutrition and exercise support programme which aims to delay or prevent the onset of type 2 diabetes.
Eligibility Criteria:
- HbA1c blood test has a reading of between 42-47 mmol/mol
- History of Gestational Diabetes Mellitus (GDM)
- Aged 18 or over
- Not currently pregnant
Exclusion Criteria:
- Pregnant at time of referral
- Individuals with confirmed diagnosis of type 2 diabetes
- Individuals with normal blood glucose readings on referral to service

Specialist Weight Management Programme
This is a non-surgical intensive medical management with a specialist team approach that consists of doctor with a special interest in weight management (physician or GP), specialist nurses, specialist dietitian, psychological support and specialist exercise therapists/physiotherapist.
The specialist service is for individuals with a BMI over 40 or a BMI 35+ with significant co-morbidities and generally lasts for a 12 month period.
If you are thinking about bariatric surgery, this in an important part of the required preparation. Your GP practice will need to refer you into this service. You will need to have undertaken either a Tier 2 programme or the National Diabetes Prevention programme before you can access the specialist weight management programme.
NHS Digital Weight Management Programme
If you are living with obesity (BMI 30+ or BMI 27.5+ for Black, Asian and ethnic minority backgrounds) and have diabetes or high blood pressure (or both), you could benefit from the free 12 week NHS Digital Weight Management Programme. Available on your smartphone, tablet or computer, the programme helps you make more informed choices, create healthy eating habits, be active and lose weight.
