Social Prescribing Link Worker
Salary: £18,469 – £20,955

On behalf of Warrington Primary Care Networks, LiveWire Warrington are seeking to appoint Social Prescribing Link Workers.
Social Prescribing Link Worker roles are funded for a period of 18 months with the possibility of an extension.
We are looking for energetic employees who have the ability to engage with a wide range of individuals, supporting them to embark on lifestyle changes that make a difference to their wellbeing. Successful candidates will work in partnership with GP practices, patients and other partners across Warrington.
Actively engaging with the local community, you will be required to have extensive knowledge of facilities and activities (assets) within the local area as referral options for patients.
Identifying any gaps in delivery, you will be expected to support local community groups in accessing funding opportunities to establish new groups and workable solutions.
This is an exciting opportunity to be a part a new programme of social prescribing across our town.
